APPOINTMENTS- Scheduling your own appointment is quick and easy through our online scheduler powered by Acuity Scheduling.
REGISTRATION IS REQUIRED- If you are being asked to register your information EACH time you shedule an appointment, it is because you have not completed initial registration.
APPOINTMENTS REQUIRING COMPLETION OF PAPERWORK
Please keep in mind when scheduling your appointment, if you are needing any paperwork completed such as FMLA (Family Medical Leave Act) to be off work, etc. or any kind of Disability paperwork, this requires more time.
For FMLA paperwork, allow 20 additional minutes to your regular scheduled appointment. There is an additional charge.
For Disability paperwork, allow 40 additional minutes to your regular scheduled appointment. There is an additional charge.
If you do not allow for extra time for paperwork to be completed when making your appointment, you will be asked to return for another appointment; one which allows adequate time for paperwork completion.
For School Notes/letters can be less involved. We ask for your keen judgment when making your appointment.
***PLEASE NOTE: ALLOW ONE WEEK FOR ALL PAPERWORK TO BE COMPLETED. If the provider completes sooner than one week, you will be notified. Thank you for your consideration.
CONVENIENT TEXT MESSAGE REMINDER
You will receive a text message 24 hours before your scheduled appointment for a reminder. This reminder is a courtesy of Psychiatric, Health & Wellness. Your scheduled appointment is your responsibility and although we provide a courtesy reminder, it is still your responsibility to arrive promptly at your scheduled appointment.
